This medal was originally struck in the early 1800's, by the U.S. Mint, at the request of the state of Pennsylvania, to honor the Pennsylvania Volunteers who were serving under Captain Oliver Hazard Perry during the War of 1812 Battle of Lake Erie, in September, 1813.  The engraver is Moritz Furst.

This is the modern U.S. Mint re-strike which, has not been available from the Mint since the 1980s.   Struck in bronze, the medal is 59 mm (about 2 3/8") in diameter.
